Greetings Warriors!
Prior to your arrival to campus, ALL students must provide the University with their current address, emergency contact, insurance, medical alert, health history information, and athletes need a current physical examination. If you are not participating in athletics you do not need a current physical, but must complete the full health history section on Sportsware. To expedite this process Waldorf University uses an online data entry system: SportsWareOnLine. Each year, you will need to update your information on your profile in SportsWareOnLine before the beginning of the year. You can find the link to Sportsware below as well as an FAQ to help you get started.

I have never used SportsWare before. How do I set up a new account?
- Go to www.swol123.net
- On the right side of the screen, under “ATHLETE/PARENT”, Click the “Join SportsWare” button.
- Next, enter your School ID. The School ID for Waldorf University is “Waldorf”. Then Click the “Next” button.
- Next, Enter the following information to request an account.
- Student’s First Name
- Student’s Last Name
- Student’s Date of Birth
- Choose “No” for register as a parent.
- Student’s Email Address”
- Choose whether you are a “Student” or “Student Athlete”
- After entering the information, click the “Next” Button.
- Click the “Finish” Button to confirm your information.
- You will then receive a pop-up message confirming that your information is received and that you will receive an email to set up your password.
- Once your request is accepted, you will receive an email with the subject “SportsWareOnLine Password Request”. Open the email and click the password reset link to continue to SportsWare or follow the directions below on how to set a password. If you do not receive this email, please check your spam folder.
How do I set up or reset my password for SportsWare via web browser?
- Go to www.swol123.net.
- Under “SIGN IN” Enter your email address and click the “RESET PASSWORD/FORGOT PASSWORD” link.
- Once you click the “RESET PASSWORD/FORGOT PASSWORD” link, you should see this pop-up window. You may not see this message if you have a pop-up blocker enabled.
- If you see the message “The email address was not found in SportsWareOnLine make sure it is typed correctly and try again. Be sure you are using the same email as when you requested to join SportsWare. If you are still seeing this error, contact Nurse Mary to see if she has accepted your account request.
- You will then receive an email with the subject “SportsWareOnLine Password Request”. Open the email and click the password reset link to continue to SportsWare. If you do not receive this email, please check your spam folder.
- Once you click the reset link, enter your email address, new password and confirm the password.
- Click the “SAVE” button.
How do I update my health information via web browser?
- Go to www.swol123.net.
- Under “SIGN IN” enter you email address and password. Click the “Login” button.
- You are now on the dashboard of the Athlete Portal. Your page may look different depending on what information/forms you need to complete. Go to the main section of your Athlete Portal to begin inputting your information for the first time. There is will tell you what forms you need to upload (if any) and what sections of your profile are incomplete. Click on the blue forms and words to be sent to the areas that need your attention.
- If you wish to explore your portal further, you can go to the various tabs for the main menu on the left side of your portal. If you cannot see the main menu to the left, click
to reveal it. - My Info is where you will put information on your demograpics, sport, address, emergency contact, insurance, medications, medical alerts, immunizations and other paperwork.
Med History is a medical history questionnaire.
COVID-19 is a place where you can take a coronavirus symptoms and possible exposure survey.
Forms is a tab where you can view/complete required paperwork. Sportsware will also display the number of forms you must complete
Print is where you can print your info and medical history data.
- All forms will show complete once you have finished uploading all required information.
How do I download the SportsWare App on my phone?
- Go to the Google Play or Apple App Store.
- Search: “sportswareonline”
- Download the SportsWareOnLine app by Computer Sports Medicine, Inc.
How do I update my health information via the SportsWare App?
- Open the SportsWare App on your phone.
- Enter your email address and password, then click the “SIGN IN” button.
- You are now logged into SportsWare. Your home page may look different depending on what information/forms you need to complete. Click on your picture to update your personal information.
- Click through the various icons to enter your contact, insurance and medical information. Then click the “SAVE” button.
- The icons correspond to the following sections of your profile.
- When you have completed your session and all information is uploaded, be sure to logout of SportsWare by clicking the button that in the upper right corner of the app, that looks like the following.
How do I reset my password for SportsWare via the SportsWare App?
- You must have an active SportsWare account before being able to use the SportsWare App. Please make sure you use a computer to setup/activate your account before attempting to use the app.
- Click the “Forget Password” link.
- Enter the email you used to join SportsWare, and click the “SEND” button.
- You will then receive an email with the subject “SportsWareOnLine Password Request”. Open the email and click the password reset link to continue to SportsWare. If you do not receive this email, please check your spam folder.
- Once you click the reset link, enter your email address, new password and confirm the password.
- Click the “SAVE” button.
- Then close your web browser and reopen the SportsWare App. You may now sign into your SportsWare account through the SportsWare OnLine App.
